We do not use advertising cookies or tracking pixels on the Site. Please do not include health, patient, payment or other sensitive information in the Notes field.

To prepare a proposal and communicate with you about the services you asked for
To provide services and manage our relationship if you become a client
To keep business records, protect the Site from spam and abuse, and comply with the law

3. How we share your information

We do not sell or rent your personal information, and we do not share it for targeted advertising. We share it only with:

When you submit the form, you agree that we may contact you by phone, text message or email about your request. We send marketing messages only if you separately opt in. You can opt out at any time by replying STOP to a text, using the unsubscribe link in an email, or contacting us.

5. How long we keep it

If you do not become a client, we keep your request information for up to 24 months and then delete it. If you become a client, we keep your information for the duration of our relationship and as long as needed for tax, accounting and legal obligations.

6. Security

We use reasonable administrative, technical and physical safeguards to protect your information, and we limit access to people who need it to do their work. No method of transmission or storage is completely secure. If a security breach affects your personal information, we will notify you as required by applicable law, including A.R.S. § 18-552.
